The goal of the Community Health Program is to promote the health of the community through education and other prevention activities, screening for early detection of disease, and helping patients manage their chronic disease or disability. 

The major services of Community Health are:

Maternal and Child Health.  The goal of this program is to ensure that children get a healthy start in life.  It serves eligible women of childbearing age and children up to age 6.   The Maternal and Child Health Program includes:

  • Family planning

  • Prenatal care

  • Prenatal classes

  • Postpartum care

  • Well child clinic

  • Immunizations

  • Home visits

Nutrition Education.  Good nutrition is basic to good health.  There are few health problems that are not affected by what we eat.  The Nutrition Program includes:

  •    Individual dietary counseling

  •    Group nutrition education

  •    Cooking classes

  •   Body composition analysis

  •   Promotion of breast feeding

  •  WIC (Women, Infants & Children) Program.  The WIC Program provides nutrition education and food vouchers to  qualifying low income women and their children.

Diabetes Education.  The diabetes program supports a variety of activities to help prevent diabetes and its complications and to help those with diabetes manage their condition.  Program activities include:  

             ·        Diabetes/pre-diabetes classes
             ·       
Individual and group education and counseling
             ·       
Wellness fairs and screenings
             ·        Shoe clinics
             ·        Retinal exams
             ·       
Exercise programs
             ·        Diabetes clinics 

Fitness Center:  The Fitness Center is located in the Lapwai Clinic and is staffed part time by an exercise specialist.  It contains a wide variety of exercise equipment and is open to Nimiipuu Health clients 1:00-7:00 p.m. Monday through Thursday. 

Massage Therapy.  This service is provided by a qualified massage therapist.  The patient must be referred by a medical provider and make an appointment in advance.  This service is most often used for treatment of pain.

Community Health Representatives.  This program provides:

      ·        Regular contact with homebound patients.  The Representative may call or visit the patient, take vital signs, or teach self care techniques. 
·       
Services for patients who do not have transportation to the clinic.  This may involve transporting the patient to appointments or delivering medications or supplies to the patient’s home. 

Medication Delivery Policy
Call Community Health (843-9375) 24-48 hours in advance
Patient must be available to receive medications.  They will not be left with anyone else.
Medications will only be delivered between 9 a.m. and 3 p.m. Monday-Friday.

Transportation Policy
Call Community Health (843-9375) 24-48 hours in advance
One week notice is needed for transport to Spokane
Transportation is for health related appointments only.  Side trips will not be allowed.
All alternative transportation resources must be used first.
Transportation will only be provided between 9 a.m. and 3 p.m.
Children must be accompanied by an adult.
Patients under the influence of alcohol/drugs will not be transported.
Seats belts must be worn at all times.

Public Health Nursing.  Public health nurses provide nursing care in a community setting.   These services include:

      ·        Working with hospitals when a patient is ready to go home.  They may help in obtaining equipment, referring you to appropriate community resources, or teaching self care. 
·       
Helping patients learn to manage their chronic health problems.
·       
Making health assessments in the home.
·       
Monitoring high risk elderly.

Community Health Education.  All programs of Community Health participate in community health fairs and other opportunities to:

  •  Teach ways to maintain good health and identify risk factors.

  • Provide health screenings.

  •   Inform them about how to access care when it is needed.

  • Encourage patients to follow their doctor’s advice.
